To receive the official 46Reader patch, children must read a minimum of 46 grade-level books during grades 4-6, with completion confirmed by a parent/guardian or teacher or librarian. Each reader should print out - fill out - and mail out this form with $1 for postage and handling to: 46Reader, C/O The Adirondack Kids, 39 Second Street, Camden, New York 13316.
